Complete Your Paperwork Online Before Your First Visit

Once your first appointment is scheduled, we send you a link to our patient portal, where every new patient form is completed online. Fill it out from your phone or your computer, whenever it suits you, and arrive ready for care instead of a clipboard.

Paperwork handled ahead of time means your first visit starts with a conversation instead of a stack of forms. The portal walks you through your medical history, your dental history, and your insurance information, and you can stop and pick it back up whenever you like. Answer it honestly, even the parts about how long it has been since your last cleaning. We meet you where you are, with zero judgment.

Sending your forms early also gives our team time to work before you arrive. Our insurance coordinator can verify your benefits and prepare your estimate, and your hygienist and dentist can review your health background ahead of your appointment. Need records from a previous dental office? Ask our front office team for a Release of Records form, and we will handle the request from there.

Dr. Shannon Morey

DMD

A Huntsville native and Auburn University graduate, Dr. Morey returned home to serve her community after earning her degree from the UAB School of Dentistry. She has a gift for calming nervous patients and building the kind of rapport that turns first visits into lasting relationships.

Tiffany, my hygienist, gave me hands down the best cleaning I’ve had in my life, thorough, careful, and genuinely pleasant to be around the whole time. She clearly takes pride in her work.Dr. Andrew Henninger is equally impressive.
